Arcoxia also known as Etoricoxib (trade name Nucoxia in India and Tauxib and Algix in Italy) is a COX-2 selective inhibitor from Merck & Co. At present, it is approved in over 70 nations across the world; however not in United States, where the FDA needs additional efficacy and safety data for Etoricoxib before it will issue approval. Current therapeutic indications include: treatment of psoriatic arthritis, rheumatoid arthritis, ankylosing spondylitis, osteoarthritis, acute pain, gout and chronic low back pain. As with other COX-2 selective inhibitor, Arcoxia selectively inhibits isoform 2 of COX-2 (enzyme cyclo-oxigenase) and reduces prostaglandins generation from arachidonic acid. Amongst different functions exerted by prostaglandins generation, their role in inflammation cascade should be emphasized. COX-2 selective inhibitors showed less marked activity on type 1 cycloxigenase unlike conventional NSAID (non-steroidal anti-inflammatory drugs). This reduced activity is the cause of reduced gastrointestinal side effects, as demonstrated in various large clinical trials performed with different coxibs.
